Two easy steps to register
Welcome to the Student Disability Services Request form!

  • Please complete the following form. You will also need to provide formal documentation of your disability.
  • Documentation Guidelines are available on the Student Disability Services (SDS) website (disabilities.uchicago.edu) or by emailing disabilities@uchicago.edu. Please note that images alone including but not limited to MRIs, X-Rays, or personal photos do not meet documentation guidelines for supporting documentation. Students are strongly discouraged from submitting images.
  • Student Disability Services may require further documentation to substantiate the request for accommodations
  • All information submitted to SDS will be maintained and used in accordance with applicable confidentiality requirements.
  • Please do not submit original copies of your documentation. All records received by SDS at the University of Chicago will be destroyed seven years after last enrollment term.
  • Communication from SDS will be prompt, using the student's UChicago email address as the primary mode.
  • This form and supporting documentation must be received before a review by SDS will proceed.

STATEMENT ON CONFIDENTIALITY

The University is committed to protecting the privacy of student education and health records with the means available by law. The University will not release information from a student’s education or health record without the student’s written consent, except in the limited circumstances permitted by law. In alignment with the University and the law, Student Disability Services (SDS) may release a student’s information to other University employees on a need to know basis. Additionally, students may request, in writing, information to be released to specific persons or agencies by signing a release-of-information form. Information contained in a student's file (including personally identifiable documentation, correspondence, and accommodation records) in SDS is considered part of their educational record and is protected under the Family Educational Rights and Privacy Act (FERPA), which is a federal law that protects the privacy of student education records. FERPA also protects student health records, including psychotherapy notes and other mental health records. In addition to FERPA, various state laws, including the Illinois Personal Information Protection Act and the Illinois Mental Health Confidentiality and Developmental Disabilities Act, also protect student health records.
